How to deal with excessive foreskin on a child’s genitals

The condition of excessive foreskin on children's genitals can be treated through observation and care, washing with warm water, local application of ointment, manual dilation, and circumcision. Excessive foreskin may be related to factors such as congenital developmental abnormalities and repeated local infections. It usually presents with symptoms such as difficulty urinating and redness and swelling of the foreskin.

1. Observation and care

For physiological foreskin hyperplasia without obvious symptoms, observation and care can be taken first. It is necessary to keep the perineum clean and dry every day, avoid wearing tight clothes, and gently wipe the glans after each urination. Parents should check the foreskin every day for redness, swelling or discharge, but it is not recommended to forcefully lift the foreskin to clean it. If slight adhesion between the foreskin and glans is found, it should be evaluated by a doctor before treatment.

2. Wash with warm water

Gently rinse the external genitalia with warm water at 38-40℃ every day, and use non-irritating baby shower gel. When cleaning, be sure to gently push the foreskin back to an acceptable range to avoid tearing caused by excessive force. After cleaning, dry the water in time and apply zinc oxide ointment and other protective agents if necessary. This method is suitable for cases of mild phimosis combined with difficulty in cleaning.

3. Apply ointment locally

When combined with postdermatitis, antibiotic ointments such as mupirocin ointment and compound polymyxin B ointment can be used as directed by the doctor, and applied thinly to the red and swollen areas twice a day. Corticosteroid creams such as hydrocortisone butyrate cream can be used to relieve foreskin fibrosis caused by chronic inflammation. During medication, the skin needs to be monitored for adverse reactions such as atrophy or pigmentation.

4. Technique expansion

A professional physician performs progressive foreskin expansion treatment in an outpatient setting, using a special dilator or fingers to slowly separate adhesions. Vaseline needs to be applied to protect the wound after each expansion. The course of treatment usually lasts 4-6 weeks. It is suitable for moderate to severe phimosis in children over 3 years old, but please note that the operation may cause painful erection or psychological fear, and local anesthesia is required if necessary.

5. Circumcision

For those with severe phimosis who have recurrent infections, difficulty urinating, or those who are over 5 years old and still cannot reveal the glans, circumcision surgery may be considered. Commonly used surgical procedures include traditional surgical resection, ligation circumcision, etc. The operation time usually takes 20-40 minutes and needs to be performed in a regular medical institution. It is necessary to keep the incision clean after surgery and use drugs such as compound zinc sulfadiazine spray to prevent infection.

Parents should be careful to avoid damaging the foreskin by forcibly turning it over on their own, and wash it regularly with warm water and keep it dry. Eat a light diet and drink plenty of water to promote urination and flushing. If you find symptoms such as pinhole-like stenosis of the foreskin opening, repeated urinary tract infections, or foreskin bulging during urination, you should seek medical evaluation promptly. During the postoperative care period, strenuous exercise should be avoided, loose cotton underwear should be worn to reduce friction, and wound healing should be reviewed regularly as directed by the doctor.
